Transcript Requests

*Any request for an official copy of your transcript or duplicate degree will require a payment of $10.00.

Please follow the guidelines below to request a transcript/duplicate degree:

1. Complete the appropriate document (see links below) in its entirety.
• Radiologic Technology/Sonography Students: complete Sovah School of Health Professions Request Form                                                                                                                                                                • Former Nursing Students: complete DRMC School of Nursing Request Form

2. Completed form can be faxed if paying by credit card to Sovah School of Health Professions- (434) 799-3718

3.
